Dadhania is a designated Civil Surgeon for the U.S. Department of
Homeland Security, U.S. Citizenship and Immigration Service (USCIS). By this
designation, she is authorized to perform medical examinations and
give necessary immunizations for those applying for adjustment of
status or Permanent Resident Status (Green Card). She also performs
examinations for citizenship.
A medical examination is necessary as a part of the application
process for an applicant applying for adjustment of status. The purpose
of the medical examination is to evaluate the applicant’s health
status and to determine if they have any conditions that might
preclude a change in status. The results of the examination will be
given to the applicant in a sealed envelope as per USCIS requirements and may also be shared with
health departments and other public health or cooperating medical
authorities.

This includes physical examination, on-premises Tuberculosis (TB) and Syphilis blood tests, Gonorrhea Urine Test and completion of current medical examination form. (I-693).
USCIS now requires all applicants, 2 years and older to have a blood test for Tuberculosis. TB skin test will not be valid.
All required vaccinations will be charged additionally at a very reasonable cost. Vaccine requirements vary based on an applicant's age, medical history, and immunization records. From October 1st through March 31st - all applicant will require to receive the current Annual Flu Vaccine. Above charges are payable at the time of service.

As of May 1st, 2024, all applicants are required to have received the Polio vaccine.
As of December 8th, 2023, all applicants are required to have received the 2023-2024 formulation of the Covid-19 vaccine.
As of April 2022, certain applicants are required to have received the Hepatitis-B vaccine.

A TB Blood test will be performed at your first visit. Blood test usually takes 5 business days for results. If you have had a positive tuberculosis blood test, you will need to have a chest x-ray to make sure you do not have active tuberculosis. This can be done at the for an additional charge. If the TB test is negative, you will not need chest x-ray.
All blood tests are administered on premises.
Certain immunizations are required by the USCIS. The requirements continually changes. If you are not able to produce acceptable proof that you have had the immunizations or the diseases themselves, you may have the immunizations administered by us or your personal physician.

examination must be paid by you at the time
of the examination.
If
you are unable to either speak or learn to
speak English and/or learn U.S. history and
government, based on a physical or mental
disability or impairment, you may qualify
for a waiver of these requirements. This medical
examination and form N-648 is intended for
applicants for U.S. Citizenship who seek and
exception to the English and civics testing
requirements for naturalization "because of
physical or developmental disability or
mental impairment".
Form N-648 allows certain applicants to apply
for partial or full medical disability
waivers based on the DSM-IV which stands for
Diagnostic & Statistical Manual of Mental
Disorders. The DSM-IV is a manual published
by the American Psychiatrist Association
which includes all currently recognized
mental health disorders that qualify for a
Medical Disability Waiver Application.
