Dr. Ketki M. Dadhania is a designated Civil Surgeon for the U.S. Department of Homeland Security, U.S. Citizenship and Immigration Service (USCIS). By this designation, she is authorized to perform medical examinations and give necessary immunizations for aliens applying for adjustment of status or Permanent Resident Status (Green Card). She also performs examinations for citizenship.

A medical examination is necessary as a part of the application process for an alien applying for adjustment of status. The purpose of the medical examination is to evaluate the applicant’s health status and to determine if they have any conditions that might preclude a change in status. The results of the examination will be provided to an Immigration officer and may also be shared with health departments and other public health or cooperating medical authorities.

We are offering Immigration Physicals for $260.

This includes physical examination, Tuberculosis (TB) Blood test, required Syphilis blood test, Gonorrhea Urine Test and completion of current medical examination form. (I-693).

USCIS now requires all applicants, 2 years and older to have a blood test for Tuberculosis. TB skin test will not be valid. Covid-19 Vaccination is mandatory as of October 1st 2021.

All required vaccinations will be charged additionally at a very reasonable cost. From October 1st through March 31st - all applicant will require to receive Annual Flu Vaccine. Above charges are payable at the time of service.

I-693 Physical Exams

What are I-693 Physicals (Green Card Physicals)?

A medical examination is necessary as a part of the application process for an alien applying for adjustment of status. The purpose of the medical examination is to evaluate the applicant’s health status and to determine if they have any conditions that might preclude a change in status. The results of the examination will be provided to an Immigration officer and may also be shared with health departments and other public health or cooperating medical authorities.

Examination Requirements

The following tests are required by the US Citizenship and Immigration Service, or USCIS (formerly the Immigration & Naturalization Service, or INS). The physical examination is required for persons of all ages. The examination requirements are determined by your age and your immunization record.

An adult examination (15 years or older) typically consists of the following:

A physical and historical examination

A blood test for tuberculosis is required for all persons 2 years or older

A blood test for syphilis (RPR) for all persons 15 years or older

A urine test for gonorrhea for all persons 15 years or older

Immunizations are dependent on age and medical history

A pediatric examination (children 14 years of age or younger) does not usually require blood testing or urine test and typically consists of the following:

A physical and historical examination

A blood test for tuberculosis (2 years old and older)

A review and update of the pediatric immunizations

A TB Blood test will be performed at your first visit. Blood test takes 3-5 working days for results. If you have had a positive tuberculosis blood test, you will need to have a chest x-ray to make sure you do not have active tuberculosis. This can be done at the for an additional charge. If the TB test is negative, you will not need chest x-ray.

New requirement for I-693 will be effective as of October 1st 2018.

Immunizations

Certain immunizations are required by the USCIS. The requirements continually changes. If you are not able to produce acceptable proof that you have had the immunizations or the diseases themselves, you may have the immunizations administered by us or your personal physician.

Adult Immunizations:

Tetanus, Diphtheria and Pertussis (Tdap) vaccine

Varicella (Chicken Pox) vaccine (If you never had Varicella Disease)

Seasonal Influenza (From October 1st through March 31st)

Pneumonia Vaccine (65 years and older)

What to Bring

Picture identification such as a passport or other Federally approved documentation (state identification such as a driver's license and school ID is not acceptable).

Your medical records for any chronic or serious medical problem,

Your immunization records, even childhood records, if available,

Alien Registration number if you have one (starts with an “A”),

Cost of exams:
We are offering Immigration Physicals for $260. This includes physical examination, required syphilis blood tests, TB Blood test, urine test for Gonorrhea and completion of required forms (I-693).

Translator if needed:
If medical records are not in English please have them translated before presenting for Medical visit. Applicants who do not speak English will be provided with a translation service at an additional cost. Please let us know if this is required when the appointment is booked.

Accepted forms of payment:
We accept cash, money order and credit cards as method of payments. There is 2.5% surcharge for all credit card transactions. All charges in connection with the examination must be paid by you at the time of the examination.

We understand the time sensitive nature of these examinations. Usually, we are able to complete the physical examination and blood work portions of the forms within two days. If you return in two or three days to have your TB skin test reviewed and have all the required immunizations completed, then you may be able to have your paperwork completed at that time.

What are N-648 Exams (Citizenship Examinations)?

If you are unable to either speak or learn to speak English and/or learn U.S. history and government, based on a physical or mental disability or impairment, you may qualify for a waiver of these requirements. This medical examination and form N-648 is intended for applicants for U.S. Citizenship who seek and exception to the English and civics testing requirements for naturalization "because of physical or developmental disability or mental impairment".

Form N-648 allows certain aliens to apply for partial or full medical disability waivers based on the DSM-IV which stands for Diagnostic & Statistical Manual of Mental Disorders. The DSM-IV is a manual published by the American Psychiatrist Association which includes all currently recognized mental health disorders that qualify for a Medical Disability Waiver Application.

Cost of Examination

Dr. Ketki M. Dadhania has professional expertise in diagnosing and evaluating an applicant for a physical or developmental disability or impairment that makes the applicant incapable of meeting the English or civics requirements for naturalization.

Charges for N-648 Examination varies depending on the complexity of cases.

Vaccine Requirements

Certain immunizations are required by the USCIS. If you are not able to produce acceptable proof that you have had the immunizations or the diseases, you will be required to get vaccines.

What is the Tuberculosis Blood test?

The Quantiferon blood test, checks for an antibody reaction in the blood of a person exposed to the tuberculosis bacteria. If test is negative then you are cleared for tuberculosis. If there is a positive test then a chest x-ray will be required to check if you have active tuberculosis. The blood tests results takes 3-5 business days.

What does a positive Tuberculosis blood test mean?

A positive blood test indicates a prior exposure to the tuberculosis bacteria. Persons with a positive test have up to 25% chance of developing infectious tuberculosis over the next several years. A prophylactic antibiotic will prevent this from occurring.

What happens if chest x-ray is required?

If the chest x-ray is normal, you do not have active tuberculosis. We will complete I-693 accordingly and this will NOT impact your immigration application at all. If you have abnormal chest x-ray then further investigation will be necessary and you will be referred to local health department for further evaluation. Till you are cleared by department of health, you cannot proceed further.

What does a medical examination entail?

The medical examination procedure consists of a history, examination, an evaluation (blood test/chest x-ray examination) for tuberculosis, urine test for gonorrhea and blood test for syphilis. The vaccination requirements include vaccines recommended by the Advisory Committee on Immunization Practices (ACIP). All these requirements vary based on applicant's age and history.

What is the purpose of the medical examination?

The purpose of the medical examination is to identify applicants with inadmissible health-related conditions for the Department of State (DOS) and U.S. Citizenship and Immigration Services (USCIS). The health-related grounds for inadmissibility include persons who have a communicable disease of public health significance, who fail to present documentation of having received vaccination against vaccine-preventable diseases, who have or have had a physical or mental disorder with associated harmful behavior, or who are a drug abuser or an addict.

What is usual turnaround time for an I-693 examination?

Usually it takes 3-5 business days to complete the process. Chest x-ray if required and time to receive blood test results may affect this time frame. If you need to complete the forms sooner then let us know and we will try to expedite everything in timely manner.

Can I open the envelope?

When medical examination is completed, you will be given a sealed envelope containing the completed I-693. Do not open this envelope in any case. Each applicant will have his or her own envelope. Take or send the sealed envelope to the INS office. If for any reason the sealed envelope is opened before the officer receives it, you will need to return the envelope to our office to have it resealed. If the INS officer receives the envelope unsealed, he will void the forms and you must redo the forms.

What are the communicable diseases that would cause an applicant to fail a medical examination or be inadmissible?

Communicable diseases of public health significance include Tuberculosis, Syphilis, Chancroid, Gonorrhea, Granuloma Inguinale, Lymphogranuloma Venereum, Hansen's Disease (Leprosy). These diseases are considered Class A conditions and your application will be denied till proper treatment is completed.

Route 70 Westbound

Take Exit #4 for Camden/Philadelphia. After paying toll keep on Left and go on to Route 73 South. Follow directions for 73 Southbound.

Take Exit #36A for Route 73 South. Follow directions for 73 Southbound.

Atlantic City Expressway (ACX)

Take Exit #31 for Route 73 North. Drive for about 20 miles and follow directions for 73 Northbound.
