I-693 Physicals (Green Card Physicals)

A medical examination is necessary as a part of the application process for an alien applying for The purpose of the medical examination is to evaluate the applicant’s health status and to determine if they have any conditions that might preclude a change in status. The results of the examination will be provided to an Immigration officer and may also be shared with health departments and other public health or cooperating medical authorities.

Examination Requirements:
The following tests are required by the US Citizenship and Immigration Service, or USCIS (formerly the Immigration & Naturalization Service, or INS).  The examination requirements are determined by your age and your immunization record and changes frequently.

An adult examination (15 years or older) typically consists of the following:

  1. History and Examination
  2. Blood test for tuberculosis for all persons 2 years or older
  3. Age appropriate Blood test for syphilis (RPR).
  4. Age Appropriate Urine test for gonorrhea 
  5. Immunizations - based on age and medical history

A pediatric examination (children 14 years of age or younger) does not usually require screening for sexually transmitted diseases and typically consists of the following:

  1. History and examination
  2. Blood test for tuberculosis (2 years old and older)
  3. Review and update of the pediatric immunizations

If your blood test for tuberculosis is positive, you will need to have a chest x-ray to make sure you do not have active tuberculosis. This can be done at the for an additional charge. If the Tuberculosis blood test is negative, you will not need chest x-ray.

USCIS now requires all applicants, 2 years and older to have a blood test for Tuberculosis. TB skin test will not be valid.

Certain immunizations are required by the USCIS. The requirements continually changes. If you are not able to produce acceptable proof that you have had the immunizations or the diseases themselves, you will be required to have appropriate immunizations administered.

Adult Immunizations

  • Measles, Mumps, Rubella vaccine (Born after 1957)
  • Tetanus, Diphtheria and Pertussis (Tdap) vaccine
  • Varicella (Chicken Pox) vaccine (If you never had Varicella Disease)
  • Seasonal Influenza (From October 1st through March 31st)
  • Pneumonia Vaccine (65 years and older)
  • Covid-19 vaccination

Childhood immunizations depend on age and may include:

  • Rotavirus vaccine
  • Measles, Mumps, Rubella vaccine
  • Tetanus, Diphtheria, Pertussis vaccine
  • Polio vaccine
  • Haemophilus Influenzae Type b vaccine
  • Hepatitis B vaccine
  • Hepatitis A vaccine
  • Pneumococcal vaccine
  • Meningitis vaccine
  • Covid-19 vaccination

What you need to bring with you: 

  1. Picture identification such as a passport or other Federally approved documentation (state identification such as a driver's license and school ID is not acceptable).
  2. Your medical records for any chronic or serious medical problem,
  3. Your immunization records, even childhood records, if available,
  4. Alien Registration number if you have one (starts with an “A”),

We understand the time sensitive nature of these examinations. Usually, we are able to complete the physical examination and blood work portions of the forms within 3-5 business days.

Translator if Needed:
If medical records are not in English please have them translated before presenting for Medical visit. Applicants who do not speak English will be provided with a translation service at an additional cost. Please let us know if this is required when the appointment is booked.

Cost of exams:
We are offering Immigration Physicals for $260.  This includes physical examination, required syphilis blood tests, Tuberculosis blood test, urine test for Gonorrhea and completion of required forms (I-693).

All required vaccinations will be charged additionally at a very reasonable cost. If you have medical insurance, vaccines may be administered by your physician. Above charges are payable at the time of service.

We accept cash, money order and credit cards as method of payments. There is 2.5% Transaction charge for all credit card transactions. All charges in connection with the examination must be paid by you at the time of the examination
